What are the 5 Phases of the Bariatric Diet?

Are you considering weight loss surgery? Are you looking guidance on how to successfully navigate the bariatric diet? 

Well in this guide, we’ll take you through the five phases of the bariatric diet and provide expert tips on what to eat, when to eat it, and why it’s important for your long-term health and weight loss success. 

Hi, my name is Ken: I’m a Gastric Sleeve Patient and to date, I’m down 300lbs. I was sleeved back in 2014. At the time of this recording, I’m actually recovering from skin removal surgery but that’s another video for another time-

 I’m here now to share what I’ve learned over these past 8 years. I just want to provide as much information as possible so you don’t have to go through this process alone. 

These are general guidelines- please consult your doctor and program for more specific guidelines and please, always follow your program- this is a process- follow the process and you’ll go far!

Phase 1: The Clear-Liquid Diet

Duration: While in Hospital

This stage usually lasts for the first 2 to 3 days after surgery, sometimes lasting for up to a week if you’re experiencing complications. 

It allows your care team to assess your tolerance for liquids and helps you learn how to hydrate yourself during recovery. 

Dehydration is one of the more common complications after weight loss surgery. 

With your new restriction,

it becomes a little more difficult to consume the 64 ounces of fluids you’ll need each day. 

This is why you’ll be hooked up to fluids until you’re discharged

Take this time to sip slowly but sip often. 

Trust me, you don’t want to go to the ER for dehydration.  It takes some practice and some patients but in time, you’ll get the hang of sipping fluids like a pro

SO Nutritional Goals for Phase 1 

In this phase, the goal is to sip 1 ounce of fluid over 15 minutes. Which means taking only small sips slowly but often

Examples of clear liquids:

  • Water
  • Ice chips
  • Broth
  • Sugar-free gelatin and popsicles
  • Calorie-free, non-carbonated, caffeine-free or decaffeinated beverages Basically like a vitamin water or gatorade zero
  • Diluted apple juice
  • Lemon water
  • Vegetable broth
  • Lemon-Lime Gatorade G2
  • Pedialyte Popsicle

Phase 2: Full Liquid Diet

Duration: Starts about 7 days after surgery and lasts for up to 2 weeks

Once you are fully tolerating liquids, this stage is designed to maximize your fluid intake. 

This is also the time when you should crack open those protein shakes you’ve heard so much about. 

Again, just like in phase one- take small sips slowly but often. 

Its a good rule of thumb to aim for 64 ounces of fluid per day which may not seem like a lot until you’re sipping from one of these cups throughout the day with a brand new level of restriction. 

Be careful when you do start sipping protein shakes as well. 

Obviously protein shakes are a lot thicker then water- the protein will also cause you to feel fuller faster so just keep that in mind while on phase 2. 

Your nutritional goal for phase 2 is to hit around 60-80 grams of protein per day while getting in 64 ounces of fluid as well.

 

Examples of things you can have on Phase 2 include:

  • All Foods in Phase 1
  • Greek or non dairy yogurt
  • Protein Shakes
  • Oatmeal or cream of wheat watered down
  • Unsweetened applesauce
  • Black beans, lentils, or fat-free refried beans (blended)
  • Pea soup, cream of mushroom soup, etc.
  • Mashed banana
  • Cottage cheese (add a little baby food for flavor)
  • Fat-free or 1% milk, soy milk, Lactaid milk
  • Low-carb, light yogurt
  • Lentil or Spilt Pea Soup
  • Sugar-free pudding (thinned-out)
  • Reduced fat cream soups (no tomato)

Phase 3: Puree Diet

Duration: Starts on day 21 after surgery, lasts during 5 and 6 weeks

This is usually exciting to most patients. Its when you can start to reintroduce some familiar foods into your system. I can’t tell you how wonderful that first bowl of pureed veggie chili tasted. It was like eating for the first time. It was wonderful but it was also very of short lived. I could only have a couple of spoonfuls at first. Going from fluids and jello to now something with texture and substance at first was very rough. Please try to remember not to force yourself to clear your bowl at this stage. It’s actually helpful to be served in something like a mug- eating from a mug just isn’t as appetizing as from a bowl and leaving food in the cup seemed a lot more doable for some reason. 

Your Nutrition goals are the same as Phase 2: You want to aim for 60 to 80 grams of protein per day and 56 to 64 ounces of fluid per day

 

Examples of Food on Stage 3 Include

  • All foods in phases 1 and 2
  • Small Curd fat-free or low-fat cottage cheese
  • Pureed beef, chicken, turkey, pork, fish
  • Fat-free or low-fat refried beans
  • Tofu
  • Pureed lentils and peas
  • Hummus
  • Fat-free or low-fat ricotta cheese
  • Canned tuna or chicken mashed with a fork
  • Mashed potatoes
  • Pureed fruit, such as no sugar added applesauce
  • Cooked and mashed vegetables
  • Baby food

Phase 4: Soft Food Diet

Duration: Starts at the 4-5 week mark after surgery, lasts for about 2 weeks.

This is usually at the [insert time] mark. This is when you can once again incorporate more foods and soft textures back into your diet. You can wean yourself off of protein shakes and powders if you’re opting for protein based foods like eggs or chicken. However, and this is completely normal- if you can’t stomach enough soft foods, stick with what’s working until you can tolerate enough food. Protein shakes are a great tool that I still use to this day. 

Phase 4 Nutritional Goals: 60 to 80 grams of protein per day and 64 ounces of fluid per day

Examples of Phase 4 Foods:

  • Lean proteins like slow cooked chicken or pork
  • Canned chicken, tuna, or crab
  • Egg whites
  • Chili or stew
  • Salmon, trout, or other flakey fish
  • Refried beans, mashed lentils, chickpeas, and tofu
  • Soft, low-fat cheeses
  • Over-cooked vegetables like zucchini, squash, cauliflower, or mushrooms
  • Soft fruits like melons, peaches, and avocados

Phase 5: General Maintenance Diet

Duration: Starts on day 50 after surgery, lasts the rest of your life

After you’ve completed your transitional diet- you are now free to reintroduce a variety of textures while still prioritizing protein and fluids above all else. Continue to focus on proper portions, eat protein first and avoid high fat, high sugar, and high carb foods. You have to make the best out of what little stomach you have. 

Phase 5 Nutritional Goals: 60 to 80 grams of protein per day and 64 ounces of fluid per day I’m sure you’re sick of hearing that- 

On this phase, you’ll want to Avoid simple sugars, which are found in candy, cookies, ice cream, table sugar, doughnuts, cake, honey, syrup, pies, sherbet, jelly, and fruit juice.

Avoid carbonated beverages. They contain refined sugar and will be counterproductive in your weight reduction goal. Coffee should be avoided as it is acidic and may cause gastric reflux.
